Life Insurance:
Group Life Insurance is offered for University employees through The Standard. The University provides Plan 1A coverage for employees in benefit-eligible positions in an amount equal to their pay up to $25,000. Additional coverage is available at the employee’s cost.
Employee Insurance:
University Provided Plan 1A Insurance - All Employees in Benefit-eligible Positions
- An amount equal to your annual salary rate up to a maximum of $25,000.
Elective Coverage
- Plan 1B – Basic Employee Life
- Life insurance on the employee’s life in an amount equal to Plan 1A.
- Plan 2 – Employee Supplemental Term Life
- Coverage in the amount of $20,000 up to $500,000 (or 5 times annual salary up to $1,000,000) in increments of $5,000.
- Premiums are based on age and tobacco use.
Family Member Insurance:
- Plan 1 – Family Member Life
- Life insurance in the amount of $2,000 on the employee’s spouse or domestic partner, and each unmarried dependent child under the age of 26.
- Plan 2 – Spouse or Domestic Partner Supplemental Term Life
- Coverage on the employee’s spouse or domestic partner in the amount of $20,000 up to a maximum of $250,000 (in increments of $5,000).
- Premiums are based on age and tobacco use.
- Plan 2 – Dependent Child Supplemental Term Life
- Coverage on each of the employee’s dependent children under age 26 in the amount of $5,000 or $10,000.
- Monthly premium is $0.45 for $5,000 in coverage or $0.90 for $10,000 in coverage.
Beneficiaries:
- Select at least one primary beneficiary and at least one contingent beneficiary for each insurance. Employees are automatically the primary beneficiary of life insurance on their spouse or domestic partner and children.
- Change beneficiaries at any time through UBenefits.
How to Enroll:
- Initial Enrollment Period – newly eligible employees may enroll through UBenefits during their first 90 days following date of hire
- Coverage begins the day after you enroll.
- Employees may enroll in Basic Employee Life, Basic Family Member Life, Employee Supplemental Term up to $350,000, and Spouse or Domestic Partner Supplemental Term up to $30,000 with guaranteed issue (no proof of good health is required). See Other Enrollment below to apply for additional amounts of coverage by providing proof of good health.
- Open Enrollment – employees may enroll in Basic Employee Life and Basic Family Member Life during open enrollment. Coverage is effective on the following July 1st, the first day of the new plan year. Employees may also increase their Plan 2 supplemental life insurance up to $20,000 or to the guaranteed issue maximum of $350,000, whichever is less.
- Status Change Event – enroll through UBenefits within 90 days of the date of the event.
- Employees who marry or newly qualify as a domestic partnership may enroll in Basic Employee Life, Basic Family Member Life and Spouse or Domestic Partner Supplemental Term Life up to $30,000 with guaranteed issue.
- Employees who add a child through birth, adoption, foster care, or legal guardianship may enroll in Basic Employee Life, Basic Family Member Life, and Plan 2 Dependent Child Supplemental Term Life with guaranteed issue
- Other Enrollment
- To enroll in Plan 2 Employee and/or Spouse or Domestic Partner Supplemental Term Life after the Initial Enrollment Period or for additional amounts, complete the underwriting application.
