WORK-RELATED INJURY OR ILLNESS
All employees of the University are covered by workers compensation insurance in the event of a work-related injury or illness.
Employer's First Report of Injury or Illness Form
Coverage Highlights:
- Workers compensation insurance pays the cost of medical expenses incurred as a result of the work-related illness/injury.
- After an employee has been off work as a result of a work-related illness/injury for three days, workers compensation pays 2/3 of income replacement.
- Report the injury or illness to your supervisor and to the Absence Management Team Human Resources.
In the event of a Work-related Injury/Illness:
- Report the injury or illness to your supervisor and to Human Resources
- Seek appropriate medical attention.
- For a life-threatening injury or illness, call 911 or go to the nearest emergency medical provider.

- Do not disturb the accident site until it has been investigated and released. Tools, equipment, materials or other evidence that might pertain to the cause of the accident must not be removed or destroyed until authorized by the Labor Commission or one of its compliance officers.
- Complete the Employer's First Report of Injury or Illness Form and submit it to the Absence Management Team within 24 hours by emailing AbsenceManagement@utah.edu.
- If the injury or illness is disabling or serious, including overnight hospitalization, broken bones, loss of limb, loss of consciousness, deep cuts, electric shock, sight impairment, or a fatality, the employee or supervisor must contact the University’s Occupational and Environmental Health and Safety (OEHS) office immediately. OEHS must notify the Utah Occupational Safety and Health Division within 8 hours of the incident.

• Notify your care provider that the University of Utah worker’s compensation carrier is Worker's Compensation Fund (WCF) and give your provider the information you gave your employer regarding the accident.
• Write your name and social security number on all medical forms the same way it appears on all of the University's payroll information.
• Ask your physician to immediately send all relevant information to Workers Compensation Fund.
• Complete and have your supervisor sign the Employer’s First Report of Injury or Illness Form.
• Submit the form to the Absence Management Team within 24 hours. Email: AbsenceManagement@utah.edu
• Inform your supervisor of the outcome of your treatment.
• Provide any note releasing you from work and returning you back to work to your supervisor and to the Absence Management Team.
• Inform your WCF claims representative, the Absence Management Team, and your supervisor of any changes or updates with your treatment, anticipated return to work date, or mailing address.
• Medical services beyond your initial treatment may need pre-authorization (check with your WCF claims representative or the Absence Management team).
WCF works to protect injured workers and their employers against fraud. It is a felony to collect workers compensation benefits through fraudulent pretenses, misrepresentations or omissions
• Assist the employee in seeking appropriate care
• If the injury or illness is disabling or serious (including overnight hospitalization, broken bones, loss of limb, loss of consciousness, deep cuts, electric shock, sight impairment), contact the University's Occupational and Environmental Health and Safety (OEHS) office immediately.
• Assist the employee in completing the Employer's First Report of Injury or Illness Form and submit the completed form to the Absence Management Team within 24 hours. Email: AbsenceManagement@utah.edu
• If the employee will miss work time because of the injury/illness, notify the Absence Management Team immediately. If the employee meets the eligibility requirements for FMLA leave, based on the workers compensation information received, Absence Management will designate the lost time as FMLA time and will notify the employee.
• Work with the employee's payroll reporter to report sick leave, personal preference and vacation time for work time not paid by WCF. Absence Management will provide instructions.
• Communicate with the employee and the Absence Management team weekly regarding the status of the employee until the employee returns to full duty or reaches maximum medical improvement.
